providers, insurance companies, or government agencies.


We may also use PHI for purposes of treatment, payment, and healthcare operations.


Treatment includes sharing information with other healthcare providers involved in a participant’s care. Payment includes sharing information with insurance companies and other
entities that assist with payment for healthcare services.

Healthcare operations include activities
that support our mission and business, such as quality improvement, patient safety, and healthcare management.

Protection of PHI


We take appropriate measures to ensure the confidentiality and security of PHI, including
administrative, physical, and technical safeguards. We train our staff on the importance of
maintaining the privacy and security of PHI and require them to sign confidentiality agreements.


Patient Rights


Participants and their parents or guardians have the right to access and receive a copy of their PHI, as well as to request that we amend their PHI if they believe it is inaccurate.

Requests for such access should be made through the participating institution, not directly to Medical Decision Logic, Inc. Participants and their parents or guardians may also request that we restrict the use or disclosure of their PHI, although we are not always required to comply with these
requests.


Complaints


If a participant or their parent or guardian has a complaint about how we handle their PHI, they may contact us using the information provided below.

We will take appropriate steps to investigate and resolve any complaints.
